How to refer someone to the project

For the Eco-op day service please contact the Kingston Eco-op Operations Manager to check availability and charges for attendance. We can discuss support needs and answer any queries regarding our service and the referral process and informal visits to our service can also be arranged.

Referrals are often completed by a health or care professional and we can accept self referrals from family members. Funding for a place must be agreed before the person can start attending their chosen activities. After the initial referral is received a trial day will be offered and if the person likes Eco-op we ask for some further forms to be completed and a Service Agreement will be drafted and signed off prior to the start date.

Step 1 - Either contact Leeni or fill out a referral form, link below.

Step 2 - After receiving the referral form, Leeni will contact you to discuss the referral. If the person would like to visit we can arrange for that to happen.

Step 3 - Once funding has been sourced, we can offer a trial day.

Step 4 - If the project is a good fit, there is a second needs assessment form which helps us get to know the support that someone needs.

Step 5 - At this point we can arrange a start date.

Frequently asked questions

Yes. Referrals can be made by health or social care professionals or by family members.

If the person has a social worker please contact them for further information. If not personal funding streams can be used.

The person will usually sign up for identified days of attendance. We offer a certain amount of flexibility if they cannot attend any particular session/s. 

For self funders we offer a bespoke Service Level Agreement for non-attendance. Usually 2 weeks for holidays and 2 weeks for sickness across any financial year.

There is a 4 week cancellation policy.

Yes, days can be changed with prior agreement with the manager.
